Sri Lankan Nationals living in Singapore

Applicant should come in person to submit the application

Payment should be made ONLY in CASH at the counter

Application Fee:

SGD 507 (with copy of lost Passport)

SGD 642 (without copy of lost Passport)

The following original documents with one set of photocopies should be submitted. Originals will be returned after due verification.

Statement (Letter) made by the Applicant regarding the lost of Passport
A Police report obtained from a local Police Station
A Copy of the Previous Passport
Original Birth Certificate
If the applicant was born outside of Sri Lanka, the Citizenship Certificate
If the applicant is above 21 years old who was born outside of Sri Lanka, please download the duly filled consent form
Singapore/Brunei SPR/EP/”S” Pass/DP/LTVP/Student Pass/WP

If the profession needs high level of qualifications, e.g., Doctor, Engineer, Accountant, Quantity Surveyor, etc… The applicant must provide educational qualification or Professional Membership Status.
For other Executive Professions, a Service Letter should be provided
For unskilled jobs e.g., labour, house maid no document is required
If the profession is already in the previous document, no document is required.

3 (Three) Passport size (3.5cm x 4.5cm) colour photographs taken within 3 months and it should not be gloss or semi-gloss
The background and dress should not be in same colour
The face must be clearly visible without any makeup and other effects
Both ears and forehead must be clearly visible
The photo should show the applicant’s eyes clearly without spectacles

Both forms must be filled and submitted by the applicant.
